Online Absence Make Up Form

Please enter the following information. In order for you to receive credit for your absence from class, the following conditions must be met:

  • You must select your correct class period
  • You must select your correct class level (Orch 1, Orch 2, etc.)
  • You must enter your name correctly (Last Name, First Name)
  • All entries for the practice period must be submitted in a single practice record
  • Your practice record must be submitted no later than 1 week after you return to school from your absence.
  • Only excused absences will be allowed to be made up.
